An Fhoirm NVB1 – Tá an fhoirm seo le líonadh agus le síniú má tá tú sásta nasc
a fháil ar ríomhphost ón Biúró Náisiúnta Grinnfhiosrúcháin. Líonann tú do
shonraí pearsanta ar an bhfoirm atá sa nasc. Téann do shonraí pearsanta díreach chuig an mBiúró ar chóras ríomhaireachta leo.
An Fhoirm NVB2 – Tá an fhoirm seo le líonadh in áit an fhoirm NVB1 má tá tú ag iarraidh do shonraí phearsanta a scríobh ar fhoirm atá i leagan scríofa. Clóscríobhann ionadaithe ó CONCOS do shonraí phearsanta ar chóras Grinnfhiosrúcháin an Gharda Síochána.
Tá an fhoirm NVB1 nó NVB2 le líonadh agus le síniú ag gach duine atá os cionn 16 bliain d’aois atá ag lorg Grinnfhiosrúchan an Gharda Síochána.
An Fhoirm NVB3 – Tá an fhoirm seo le líonadh agus le síniú ag tuismtheoir/Caomhóir ar son an té atá os cionn 16 bliain d’aois ach atá faoi 18 bliain d’aois. Tugann an fhoirm sínithe seo cead don Biúró, Grinnfhiosrúchán a dhéanamh ar an aoisghrúpa seo.
Fianaise ar aitheantais
Nuair a sheoltar ar aghaidh d’iarratas, caithfidh fianaise ar aitheantais de luach 100 pointe a bheith i dteannta leis. Seo chun cinntiú go bhfuil Grinnfhiosrúchán an Gharda Síochána á dhéanamh ar an duine atá ainmnithe ar an bhfoirm NVB1 nó NVB2. (Is leor cóipeanna den fhianaise). Tá fianaise ar aitheantais ag teastáil ón duine atá ag líonadh agus ag síniú an fhoirm NVB3, chomh maith.

Seolfaidh CONCOS ar aghaidh na Grinnfhiosrúcháin a thagann ar ais ón mBiúró chuig Roinn an Gaeltachta (i gcás na dteaghlaigh Ghaeltachta) agus chuig an gColáiste Gaeilge atá luaite ar an bhfoirm.

Garda vetting has to be completed by any person who is living in the house of a Gaeltacht hosting family, who is assisting in the home or who is a regular visitor to the home. Garda vetting must be completed by persons who will be employed by the Irish College, including Managers, Principals, Teachers, Assistants, Prefects and Bus drivers.
Form NVB1 – This form is to be completed and signed if you are happy to receive a link directly through email from the Garda Vetting Bureau. You provide your personal information through an online form format. Your personal details go directly to the Garda Vetting Bureau on their computer system.
Form NVB2 – This form can be completed instead of the form NVB1 if you would prefer to write your personal details in written format. A representative of CONCOS will enter your personal details into the Garda Vetting computer system..
All persons who are over 16 years old who are required to be Garda Vetted must complete, sign and return form NVB1 or NVB2.
Form NVB3 – This form is be completed and signed by the parent/guardian of the person who is over 16 years old but under 18 years old. This signed form gives permission to the Bureau to conduct a vetting check on behalf of this age group.
Proof of Identity
Proof of Identity to the value of at least 100 points must accompany the application. This is to ensure that Garda vetting is completed on the person who is named on form NVB1/NVB2. Proof of Identity must be provided by the person completing and signing form NVB3 also.
(Photocopies are required for proof of identity)
Send original copies of form NVB1/NVB2, NVB3 and copies of Proof of Identity to:
CONCOS, Tír an Fhia, Leitir Móir, Co.na Gaillimhe
Concos will send the Vetting received back to Roinn na Gaeltachta(in the case of the Gaeltacht hosting families) and to the Irish Colleges that is mentioned on the form.
